There are a lot of brilliant players on this subreddit, but they tend to play optimally on a20 runs that incorporate challenges beyond just beating Act 3. The relics are less valuable (ie getting Old Coin with no shops left), and sometimes you just want to maintain your health and mess up the boss with some powerful potions. Frankly, I often avoid third act elites when I don't have a killer deck. I rarely lose on A0/A1, and plenty of those wins include minimal elites. It's somewhat true on higher ascensions because you need an OP deck just to survive to the end, but it's really not necessary and often detrimental on weaker low-ascension runs.
